Healthy Weight Clinic

Aaron E. Henry Community Health Services Center, Inc. (AEHCHC) has partnered with Harvard Medical School, the American Academy of Pediatrics Institute for Healthy Child Weight, and others in the Healthy Weight Clinic/Childhood Obesity Research Demonstration (CORD) initiative. The project seeks to remedy the age-old problem of childhood obesity in the Mississippi Delta region. Historically, our region has had one of the highest rates of childhood obesity and diabetes in the nation. Through this effort, AEH seeks to end that statistic. The program, which works with youth between the ages of 2 – 18 with a BMI > the 85th percentile, is an obesity treatment intervention rooted in primary care with two key components: identification and referral of pediatric patients with obesity and ongoing multi-component obesity treatment. The program will utilize individual family clinic visits, as well as group sessions with patients and their families. Participants commit to a minimum of 30 contact hours over the course of 12 months. Dr. Yasmin Cheema, Pediatrician, serves as the Site Champion and Khalilah George, FNP, serves as Medical Provider/Well Child Visit Coordinator at AEH. They will provide consultations and hands-on assistance for individual visits and group sessions. CYSHCN Services

AEH’s Children and Youth with Special Health Care Needs (CYSHCN) Services, in collaboration with the Mississippi State Department of Health, provides care coordination services for children and youth.  Care coordination is designed to help your child achieve the best possible health and wellness.  It does this y connecting children and their caregivers with services and resources that are right for their specific needs, whether they are medical, behavioral, social, or developmental.  We work with you and your child to develop a unique plan of care.  We work closely with each family to identify needs and create a plan of care just for your child.  This includes coordination with primary care providers, specialty clinics, early education programs, hospitals, and mental health services.  This program works with clients from birth – 21 years of age with one or more eligible conditions, to include asthma, autism/Asperger’s, ADHD, obesity, sickle cell disease, diabetes, hearing abnormalities, eye and vision abnormalities seizure and other neurological disorders, orthopedic disorders, and cardiac/heart disorders.  Call us at 662-624-2504 ext. 2204.
